FOLLOWING CAREFULLY BEFORE SIGNING

I am aware that certain dangers are inherent in rafting, canoeing and kayaking excursions. By my joining the Oregon River Sports on any rafting, canoeing or kayaking excursions, I certify that I am aware of all dangers, including mortal and/or severe injury, and have familiarized myself with all precautions that can be taken to prevent injury to myself, and any minors that may accompany me. ______

I understand that it is not the function of Oregon River Sports, Inc., or its officers, directors, employees, shareholders, agents and their assistants, to serve as guardians of my safety. I warrant that I am in good physical condition and know of no medical or health reasons why I should not participate in water sports activities. I have made Oregon River Sports, Inc., staff aware of any dietary or medical needs. I understand and agree that Oregon River Sports, Inc., its officers, directors, employees, shareholders, agents, and their assistant shall not be held liable in any way to me, my family, heirs, executors or assigns for any occurrence or accident which may result in injury, death, or other damages in connection with an Oregon River Sports rafting, canoeing or kayaking excursion. ______

In consideration of joining the Oregon River Sports on a rafting, canoeing or kayaking excursion at a shared cost which does not reflect any premium for insurance, I hereby personally assume all risks in connection with participating in an Oregon River Sports excursion/adventure trip, including but not limited to those associated with forces of nature, physical injury, or illness and accidents resulting from falls, and I further agree to forever release, discharge, waive, save and hold harmless Oregon River Sports, Inc., its officers, directors, equipment. This includes, but not limited to, any and all rights, demands, or claims for damages and causes for suits or actions known or unknown, and further includes, but is not limited to any and all medical expenses or other expenses and damages in the event of a accident, illness, other incapacity, death, or damage property, however caused. Notwithstanding the foregoing, it is understood and agreed that this release is not intended to release Oregon River Sports, Inc. from liability for claims for loss or damage based on gross negligence or reckless indifference of Releasee. _______

I further state that I am of lawful age and legally competent to sign this Affirmation, Waiver, Warrant, Release and Assumption of Risk for said minor(s) and myself. _________

I have fully informed myself of the contents of this Affirmation, Waiver, Warranty, Release and Assumption of Risk by reading it before I signed it. ________

I give ORS the authority to use any photos of myself or others given to ORS, and may be used for the promotion of paddle sports (on the worldwide web, brochures and newspapers). _______

I willingly sign this liablility form and
